Salon Waiver

This is an example of the information on our salon liability waiver. Terms subject to change. A physical copy of this waiver will be presented at the beginning of all appointments to be signed by the client.

By signing this waiver form, I acknowledge and agree to the following terms:

• I have been honest in disclosing any and all history of hair services involving chemical work prior to this appointment.
• I am responsible for disclosing any allergies towards chemical hair products to my service technician prior to starting the service.
• Busto Beauty will not be responsible or liable if the result of the service is not as expected as it should be.
• I will follow the after care regimen and the suggested follow-ups of the salon in maintaining and treating my hair.
• I am allowing Busto Beauty salon to apply necessary chemicals as part of the service in my hair treatment.
• I understand that the result of this chemical may vary from one person to another.
• I acknowledge that Busto Beauty employees are licensed professionals.
• I have read this whole document and I accept the terms indicated above.
